The images may be used for the following purposes:

Print publication (Annex 1)
Other uses (Annex 2)

I am aware that I will not receive any payment for the publication. I can withdraw or limit this consent about the publication and use of photos showing me at any time. Otherwise, the consent is valid indefinitely.

Prior to publication, I would like to be informed about the images concerned, so that I can object to publication for individual images in case of doubt.

I would like to obtain image rights for the following purposes:

Use on the social media platforms of (Annex 3)
Other uses (Annex 4)
